



Bilateral Stifle OCD Arthroscopy in a PRE Gelding by Dr. Jennifer Jordan and Dr. Jose J. Bras
Patient Presentation
A 4-year-old PRE gelding presented for evaluation of hind-end weakness. Dr. Jennifer Jordan examined the horse at the farm to assess conformation and soundness concerns.
Initial Evaluation and Findings
Radiographs obtained during the examination revealed large osteochondritis dissecans (OCD) lesions in both stifles. Due to the severity and size of the lesions, the gelding was referred for surgical management. Large OCD lesions can compromise joint stability and affect long-term athletic soundness if not appropriately addressed.
Medical or Surgical Management
The gelding underwent bilateral stifle arthroscopy performed by Dr. Jose J. Bras for removal of osteochondral fragments and debridement of abnormal bone. Given the extensive nature of the lesions, careful surgical technique was required to remove diseased tissue while preserving joint integrity. Following surgery, the horse recovered uneventfully from anesthesia and was discharged with a prolonged rest and rehabilitation plan. Sutures were removed two weeks postoperatively, and controlled exercise was gradually reintroduced.
Outcome
The gelding recovered well and successfully returned to routine exercise following rehabilitation. Ongoing collaboration between veterinarians, surgeons, technicians, and the owner supported a positive outcome.

Large stifle OCD lesions can pose significant challenges for surgical management and long-term soundness. Early diagnosis, meticulous arthroscopic debridement, and structured rehabilitation are essential to optimize joint stability and athletic prognosis.
